Disability Services

Application for Services

 

To receive Accessibility and Disability Services, you must be:

  1. A registered student at Minnesota West and
  2. Have a recognized disability.

Examples of Recognized disabilities are:

  • Blindness or Limited Vision
  • Deafness or Impaired Hearing
  • Mobility Impairments.
  • Neurological Disorders.
    • Multiple Sclerosis
    • Epilepsy
    • Cerebrovascular diseases
    • Parkinson’s Disease
    • ABI-Acquired Brain Injury/TBI-Traumatic Brain Injury Disability.
  • Learning and/or Intellectual Disabilities.
  • Mental Health Disorders.
    • ADHD/ADD-Attention Deficit Hyperactivity Disorder/Attention Deficit Disorder.
    • Autism
    • Post-traumatic Stress Disorder
    • Anxiety Disorders
  • Any other documented disability that could limit success in a program or activity without accommodations.

Steps to request Accessibility and Disability Services:

  1. Complete the Request for Accessibility and Disability Services form. You may also request a paper-copy from your Campus’ Student Services Advisor.
  2. Submit the form along with documentation substantiating your disability, either in person or through email, to the Accessibility and Disabilities Services Coordinator.

Documentation substantiating your disability may include:

  • Educational records (such as IEP’s, SOP’s, etc.)
  • Medical records (such as a doctor or mental health practitioner’s letter, evaluation reports, etc.)

The goal of the initial meeting is to establish a relationship with the Accessibility and Disability Services Office, discuss your needs, talk about what services are available, and set up a plan.

  1. From there on, it is the student’s responsibility to stay in open communication with the ADS Coordinator if the student’s needs change and the plan needs to be adjusted.

Accommodations are arranged each term and students need to communicate with the Accessibility and Disability Services Coordinator to arrange for academic accommodations prior to, or at the beginning of, each term.

A copy of the Minnesota State Board Policy 1.4 will be provided to any individual requesting services.
